While lowering a 67 kg concrete kerb unit with a colleague, the injured person felt a sharp pain in his lower back. The lift was the fourth in a run of six; the pallet had been set down about 8 m from the trench, so each unit was being carried over uneven ground. The mechanical kerb lifter was in the van but was not in use. He stopped work immediately and reported the injury to the site supervisor.

Immediate & root causes

Immediate cause: manual lifting of a heavy kerb unit over a long carry across uneven ground.
Underlying causes: pallet set down too far from the workface; the mechanical kerb lifter was available but not in use; the manual-handling controls in the RAMS were not followed for this task.

Corrective actions to prevent recurrence

1. Kerb units to be moved with the mechanical kerb lifter — no manual lifting of units over 20 kg (immediate — Tom Bradley).
2. Deliveries to be set down within 2 m of the workface where ground conditions allow (immediate).
3. Toolbox talk on manual handling and kerb-lifter use delivered to all operatives (completed 03/07/2026).
4. RAMS lifting-operations section reviewed and re-briefed to the crew (completed 03/07/2026).
5. Phased return on light duties planned from 13/07/2026, subject to GP advice.
